Hospice Social Worker/Bereavement Coordinator

Job Description:
Social Worker/Bereavement Coordinator works under the direction of the Director of Hospice to perform skilled social work assessment for Hospice patients, their families and caregivers in all settings. Bereavement Coordinator will include developing,documenting, implementing and evaluating the bereavement plan of care.
