RN'S/ RN Home Health Case Managers
Status: FT/PT/PD

Job Description:
Nurses:Works under the supervision of the Director of Home Health or Hospice to provide skilled nursing care to patients and to participate in the implementation of the Plan of Care and the ongoing assessment of the patients' progress toward meeting the treatment goals.

Case Managers:Works under the direction of the Director of Home Health or Home Health Manager to plan, implement, and assess the plan of care for each patient in the caseload and utilizes principles of community health nursing to provide skilled nursing care to patients.
